We are very excited to announce that the Hamady High/Middle School Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. In addition, you will be able to print the MHSAA health questionnaire or physical examination form directly from your FinalForms site and take that with you to the doctor for your child’s physical (no need to pick one up from school).

All Parents of Hamady HS and MS athletes must register their athletes. Please follow the prompts to create your account, create your athletes and sign your forms.

Pay to Participate

Hamady Middle/High School athletes are required to pay a ONE TIME $5.00 registration fee. The registration fee covers all sports played by the athlete for the current school year. An athlete must have the fee paid by the first athletic practice in his/her given sport or he/she will not be allowed to participate until the fee is paid.
